Veterans Care Coordination?
American Veterans Care Coordination, LLC (AVCC) is an independently

and management of homecare so he/she can remain in the comfort of
their home. AVCC makes sure all clients receive the hours they
purchased and are satisfied with the services. AVCC also
monitors clients to keep them in compliance with VA regulations to
avoid loss of the pension benefits.
2. What is the program and
who is entitled?
The program is available through the Veterans Administration and is
designed to assist both veterans and their surviving spouses that
suffer from non-service related medical conditions and served during
an active period of war. This program is designed to assist in
their every day needs such as:

Eligible veterans and surviving
spouses must meet specific income and liquid asset
requirements. They may then use the specially designated
benefits to purchase personal care and homemaker assistance.
3. How does it benefit
veterans and surviving spouses to use AVCC?
There is no charge for any interested veteran or surviving spouse to
file an application for VA pension benefits. AVCC will make a
personal home visit or phone assessment to obtain information on your medical
conditions, income, and liquid assets. Our staff uses our
experience in long-term care and an RN to gather any medical
information and paperwork required as evidence to complete and file
your application with the VA. AVCC will advocate on your
behalf to purchase the services you need through a network of
reputable providers. We continue to ensure your satisfaction
with those services and monitor eligibility compliance to remain on
the program through monthly contact.
4. Why should Providers
use AVCC?
Providers are able to offer other funding sources to their existing
and potential clients, therefore increasing revenue by the number of
hours a client is serviced monthly. Providers that work with
AVCC benefit by having an experienced company with properly trained
staff to identify and process eligible clients of the provider to
the program. AVCC maintains a confidential relationship with
each of its providers. Every client a provider refers to the
program will only receive services from the provider. In
addition, providers can be assured and confident they are assisting
veterans and their surviving spouses to receive all of the options
available he/she needs to remain independent at home.
